• A handwritten application addressed to the Rector requesting participation in the competition (Appendix 1);
• A consent form for the processing of personal data (Appendix 2);
• A personal record sheet with a photo and an autobiography (to be submitted by applicants applying to the University for the first time);
• Certified copies of diplomas (with supplements) of higher education and academic degrees, certificates of associate professor, senior researcher, or professor (for applicants applying to the University for the first time, or in the case of awarding a scientific degree and/or academic title during the current employment contract period at the University);
• A document confirming proficiency in the state language (a certificate of complete general secondary education or a higher education diploma, provided that the document confirms that the applicant studied the Ukrainian language as an academic subject with a grade of “good” or “excellent”; a state certificate confirming the level of state language proficiency issued by the National Commission on State Language Standards; or the University's internal certificate confirming the completion of language courses);

The second USUST Hackathon

•  a list of published scientific and educational-methodological works for the last five years, prepared according to the form provided (Appendix 3);
•  documents confirming professional development and internships completed over the last five years (a diploma, certificate or other documents provided for by the legislation of Ukraine) approved by an order of the University or another institution where the applicant works;
•  information on the types and results of the applicant’s professional activity in the relevant specialty over the last five years, which is used to recognize qualifications corresponding to the specialty (Appendix 4);
• a five-year prospective development plan for the department, submitted by applicants for the position of Head of Department (Appendix 5);
• a report on educational, educational-methodological, research, organizational, and mentoring work at the University during the term of the previous employment agreement/contract, approved at a department meeting (Appendix 6);
• an extract from the minutes of the department meeting, including discussion of the applicant and voting results (Appendix 7);
• an extract from the minutes of the staff meeting of the department, submitted by applicants for the position of Head of Department (Appendix 7);
• an extract from the minutes of the department meeting on the delivery of an open lecture/practical/laboratory class, including discussion with department members and a positive conclusion. The open class is valid for one calendar year from the date of delivery until the completion date of the competitive selection procedure;
